Speaker: Yelena Kan, RN, MSN
Yelena is a Registered Professional Nurse with over 30 years of diverse
clinical experience. Her background includes significant roles in hospital
medical-surgical and telemetry units, as well as in-patient hospice care,
where she provided compassionate, high-level patient care.
For the past 20 years, Yelena has focused on home health, serving as both
a visiting nurse and a case manager. In these capacities, she has delivered
skilled nursing services, supervised home health aides, and ensured the
continuity of care for diverse patients. Her work has consistently
emphasized the importance of matching each client’s medical needs and
personal preferences with the most appropriate caregiver, fostering both
clinical success and patient comfort.
Yelena’s leadership extends beyond direct patient care. She has acted as a
community liaison nurse and held the position of Director of Business
Development at a major New York home care agency. In these roles, she
developed and maintained strong professional relationships with
physicians, nurses, and social workers across skilled nursing facilities and
community organizations—facilitating seamless care coordination and
improving patient outcomes.
With a comprehensive understanding of healthcare systems and insurance
guidelines—including Medicare, Medicaid, private insurance, and long-term
care—Yelena is well-equipped to guide clients and families through the
complexities of home care service selection and reimbursement.
Yelena is also the founder of Izabella Home Care, a home health aide
agency dedicated to providing the highest level of care, with a mission to
support individuals in maintaining their independence and well-being in the
comfort of their own homes.
Throughout her career, Yelena’s commitment to patient well-being and her
ability to deliver thoughtful, individualized care have been hallmarks of her
professional practice.
